Does Gastric Bypass Help Type 2 Diabetes More Than Gastric Sleeve?

Doctor writing notes in a medical notebook at a desk

A 2026 study found that gastric bypass was associated with higher rates of type 2 diabetes remission than gastric sleeve surgery, and that this difference held even when both groups lost a similar amount of weight. The findings suggest that something beyond weight loss itself may be driving the benefit.

The Study Looked at 1,440 People With Type 2 Diabetes

Researchers drew on two large Scandinavian registries to study 1,440 people, 720 who had Roux-en-Y gastric bypass and 720 who had sleeve gastrectomy. All participants had type 2 diabetes before surgery. The groups were carefully matched so that factors like age, weight, blood sugar levels, how long they had lived with diabetes and how much weight they ultimately lost were comparable across both groups.

Gastric Bypass Was Linked to Higher Diabetes Remission at Two Years

At two years after surgery, about 7 in 10 people in the gastric bypass group had complete type 2 diabetes remission, meaning their diabetes was no longer measurable without medication. In the sleeve group, about 6 in 10 reached the same outcome. The gastric bypass group also had higher remission rates for high blood pressure. There was no significant difference between the two procedures in rates of remission from high cholesterol, and no meaningful difference in serious cardiovascular events.

What This Study Can and Cannot Tell Us

Because both groups lost a similar amount of weight yet still showed different outcomes, the researchers concluded that weight loss alone does not fully explain the advantage seen with gastric bypass. This was a registry based study drawing on data from Scandinavian populations, so results may not apply equally to everyone. As always, individual outcomes vary from person to person.
