In accordance with the Victorian Health Records Act 2001 and Privacy Act
Our Practice respects your right to privacy. It is important that you understand why details about your health are collected, how this information is used in our practice and to whom this information might be disclosed.
The policy of this practice is to follow these procedures:
- The information collected will be used for the purpose of providing treatment to you. Personal information such as your name, address and health insurance details will be used for accounting purposes and correspondence related to treatment.
- Details of your health information may be disclosed to other healthcare professionals, or required from them if it is necessary in the context of your treatment. In the event, disclosure of your personal details will be minimised wherever possible. Certain conditions including most cancers & some infections are reported compulsorily to government agencies.
- Parts of your health information may be used for research purposes, in study groups or at seminars as this may provide benefit to other patients. Should that happen, your personal identity will not be disclosed and confidentiality will be respected, however if you are concerned you may advise that you do not wish to be involved.
- Your record, and any other material relevant to your treatment will be retained. (In some cases for a limited period.) You may inspect or request copies of our records, or seek an explanation of your treatment at any time. Statutory fees may apply in relation to the type of access you seek.
- If any of the information we have about you is inaccurate, you may ask us to alter the record accordingly.
- Your health information will be treated with confidentiality. Disclosure will not be made to any person or organisation not involved in either your treatment or the administration of this practice without your written permission.
